Facility
We are proud of our brand new facility for the highest quality chiropractic care available. It is one of our top priorities to protect the well-being of our valued patients. I have been practicing here in Land O' Lakes since March 1983 and am very proud of the relationships I have established over years. We are confident that you will feel right at home in our office as we welcome all patients as if they were family.
Appointments
If you would like to make an appointment, please contact our office by phone at (813) 996-9800 or email at macdc@verizon.net. By phone you will either speak to my wife Linda (34 years of marriage) or Joy. Our office strives to provide convenient appointments, often on the same day you call, even for new patients seeking help. We "book out" two times each day to provide for emergency calls from new patients. Usually late morning and early afternoon. We make every effort to see each patient on time as we know your time is valuable. I hate to wait and I promise you every effort will be made to see you on time, every time. Our average wait in the reception area is 5 to 10 minutes during our busy times, which usually the first hour of the morning and the last two hours of the evening.
Downlaod the intial paperwork and fill it out in advance to reduce your waiting time on the first visit. Bring it in, or better yet fax it to us at (813) 996-3326 and we will have your file prepared when you arrive.

New Patient emergencies will be seen as soon as possible or on the next regular work day. In the case of an Auto Accident every effort will be made to see you as soon as possible, even on the weekends.
Financial
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, debit cards, or major credit cards.
Insurance Information We accept most major insurance carriers. We are a provider for Blue Cross Blue Shield PPO, but not BCBS HMO. We are a provider for most United Healthcare Plans and Cigna. We also accept HUMANA and HUMANA HMO. (most Humana Gold HMOs have a $25 copay) Usually the copay covers the first visit exam and x-ray for Humana HMOs. We are not providers for Aetna but do send claims if you have out of network benefits. We are also providers for Staywell, Healthease, and Wellcare and most Beachstreet plans and several other less well know insurance companys. To find out if your insurance company has chiropractic benefits, call the number on the back of your card and ask them specifically for chiropratic benefits or you can look on thier website. Be sure to find out what your yearly deductible and/or what your copay is on your plan.
For Auto Injuries, always call your carrier to let them know that you are seeking treatment. We will document your injuries and refer you for MRIs if necessary. Don't let that pain get the best of you. We will also work with your attorney and wait for payment when your case is settled, if necessary.
Self pay patients are always welcome. We offer a cash discount program through ChiroHealthUSA which will help reduce your "out of pocket" expenses plus provide a 50% discount for immediate family members
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
